Our Office Policies and Information

Our personal relationship with you is an essential part of helping to maintain your teeth and gums as well as your overall health. If you have any special concerns, please let us know so we can discuss them with you.

For all new patients, the doctors require a one hour consultation appointment. This includes a thorough assessment of your oral health, discussion of your prevention habits and a medical health review including medications.

Digital x-rays will be taken unless you have had a full mouth series within the last year.  If you would like to have recent xrays taken elsewhere for our review contact your previous dentist and request them to be mailed, emailed or picked up for your to hand carry to our office in time for your first visit.

Your first appointment can correspond with a dental cleaning.  Please discuss this with our receptionist when you call.

Please request Dental Xrays from previous dentist if taken in the past year.

Online forms completed no later than two days prior to scheduled appointment.

Complete billing information is needed if you are using dental insurance to cover a portion of your care.

 Financial responsibility and arrangements will be determined prior to the start of care.

All emergency dental services and any dental services performed without previous financial arrangements must be paid for in cash or credit card at the time services are rendered.

Patients who carry dental insurance understand that all dental services are charged directly to the patient and that he or she is personally responsible for payment of all dental services.  This office will assist with insurance forms, assist in making collections from insurance companies, and will credit any collections from insurance to the patient's account.  This dental office cannot render services on the assumption that the resulting charges will be covered by insurance.

We sincerely appreciate when patients keep their scheduled appointments. We require a two business day cancellation notice. Within that two day window, or if a patient does not show for an appointment, a charge for the reserved time will be billed directly to the patient.
